Advance Care Planning 

Advance Care Planning is a way to ensure there is always someone who understands a care recipient’s wishes and can help prepare for future healthcare decisions.

Advanced Care planning can help you ensure the person you’re caring for continues to receive the care they need if they are no longer able to make healthcare decisions for themselves.

As a first step, you may wish to better understand the values of the person you’re caring for. Talk to the person you’re caring for to find out more about:

Working through these questions will help to inform decisions related to their care that may come up in the future.
