In this webinar, we will cover two main topics. The first is a discussion of the fundamental aspects of advance care planning (ACP) as it applies to you and to the people you care for. We will discuss what ACP is, why it is important and how to begin advance care planning discussions. We will also discuss the role of a substitute decision-maker (SDM). The second topic will include a discussion of important concepts in caring for people with advanced illness, such as “what does resuscitation status really mean?” and “when should I have palliative care involved?” and “what is the difference between care in a hospital versus a palliative care unit or hospice?”

This webinar is presented by Dr. Leah Steinberg, a palliative care physician at Mount Sinai Hospital. She currently teaches palliative care to all level of learners and is involved in research in creating models for integrating palliative care into chronic disease management. Dr. Steinberg is also a member of the Advance Care Planning, Goals of Care, and Health Care Consent Clinician Advisory Team at Hospice Palliative Care Ontario. Dr. Jeff Myers will join Dr. Steinberg in this presentation.

About this Event

The Support for Caregiver Awareness, Learning & Empowerment (SCALE) Program is a series of free, weekly webinars for family caregivers. Facilitated by Leah Weisberg, RN, Michal Goldstein, MSW and Deborah Miller, MEd, the program aims to empower caregivers with practical information, and skills to enhance self-awareness with a focus on your own needs and well-being. Select a topic you are interested in each week or take the entire program. As a caregiver, we understand how valuable your time is and that is why we developed a program to best suit your schedule.

Week 1 Topic: Your Caregiving Journey (October 13th, 2020; 12-1pm)

Exploration of the stages of caregiving and the understanding of the care recipient’s experience.

Week 2 Topic: How Caregiving Stress Affects You (October 20th, 2020; 12-1pm)

Exploration of the physical, emotional, and mental impacts of caregiving.

Week 3 Topic: Become a Mindful Caregiver (October 27th, 2020; 12-1pm)

Explore the seven attitudes of mindfulness and how this can be integrated into your caregiving relationship.

Week 4 Topic: The Self-Compassionate Caregiver (November 3rd, 2020; 12-1pm)

Manage your inner critic through developing your soothing self.

Week 5 Topic: How Caregivers can Overcome Sadness & Guilt (November 10th, 2020; 12-1pm)

Explore the feelings that drag you down and keep reminding you that you aren’t doing enough and about the cognitive distortions that keep you in the shame cycle.

Week 6 Topic: How Caregivers can Overcome Anxiety & Anger (November 17th, 2020; 12-1pm)

Explore where feelings of anxiety and anger come from, and how to manage them in the context of your caregiving role.

Week 7 Topic: Find Your Caregiving Strengths (November 24th, 2020; 12-1pm)

Explore the top attributes and resources you already possess that enable you to succeed in your caregiving role.

Week 8 Topic: Time for Self-Care While Caregiving (December 1st, 2020; 12-1pm)

Explore strategies on how to integrate psychological and physical tools to manage your mental health daily. Based on Motivational Interviewing skills, health coaching techniques and integrative health assessment quizzes.

Biographies

Leah Weisberg (RN, BC-NC) is a Nurse Psychotherapist & Coach and Director of Interprofessional Practice at Dynamic Health Clinic in Toronto. She is a Board-Certified Nurse Coach through the American Holistic Nurses Credentialing Corporation and views each person and situation from a bio-psycho-social-spiritual lens. Leah has seen first-hand the way caregiving affects one’s physical and emotional health, and she works with formal and informal caregivers on developing personal wellness plans that allow one to be in a better place to provide care to others. Leah is passionate about showing caregivers the power that relationships can have on the well-being of individuals and has worked with many nurses looking to decrease the stress involved in caring for sick patients. Leah is Past-President of the Canadian Holistic Nurses Association and a member of the Canadian Federation of Mental Health Nurses. She lives in Toronto and spends her time advocating for an integrative approach to healthcare. She speaks to a variety of audiences about the ways in which the body and mind are connected.

Michal Goldstein (RSW) is a Registered Social Worker working at Dynamic Health Clinic in Toronto where she sees adults for individual, couples and group counselling. She is also the founder of Creating Connections, a play-based therapy practice in Toronto, where she works with children suffering from mental health and behavioral challenges. She has over a decade of experience working with caregivers to support them in improving their ability to cope with the challenges inherent in caring for someone else. Michal has developed numerous workshops and support group programs utilizing knowledge of interpersonal neurobiology and mindfulness techniques to support individuals suffering from physical health, mental health, and parenting challenges. She is passionate about bringing evidence-based practice to the forefront of the clinical social work field. Michal knows both first-hand and from professional experience, the challenges that come with caring for someone with a physical and mental health illness and is grateful for the opportunity to bring more awareness, education and support to this community.

Deborah Miller (MEd, ACC, CCDP) researches, develops, delivers, and evaluates positive psychoeducational programs, coaching, and coach education. Deborah is deeply committed to supporting organizations and direct service clients to showcase and enact strengths and create hope, optimism, and flourishing in the world of work and in the greater context of life. She is a Doctoral candidate in Coaching Psychology and holds a Master of Education in Adult Education (MEd). She is an Associate Certified Coach (ACC) through the International Coach Federation (ICF), a coaching practice leader, and a Certified Career Development Practitioner (CCDP). Deborah also acts as adjunct faculty at the post-secondary level. If we are all “just walking each other home”, Deborah believes there is a path forward for each of us to discover within that will enable us to be fully human on this great walk home.
