This education session is co-facilitated by The Dementia Society of Ottawa and Renfrew County and care-coordinators from the Home and Community Care LHIN (Local Health Integration Network). The session is intended to guide caregivers through key aspects of the decision making process as they consider whether or not to move their family member in long-term care. A special focus addresses the varied emotions caregivers may experience as they consider their needs and their options. Caregivers will also receive information on care services, including: Home care and long-term care; the application processes; and the eligibility criteria for these services.
