Caregiver Link Worker, Eastern Ontario (Bilingual – English / French)

  • Full Time, 36 Month Contract – Start Date April 2026
  • Virtual First Organization; Work from home-based office and regular in-person work and travel required in Eastern Ontario (i.e., Durham to Ottawa and Frontenac to Haliburton )
  • Salary Range: $75,000 – $80,000

The Ontario Caregiver Organization (OCO) exists to improve the lives of Ontario’s estimated 4 million caregivers; ordinary people who provide physical and/ or emotional support to a family member, partner, friend, or neighbour. OCO provides caregivers with one point of access to information, services and supports that empower and help enable caregivers to be successful in their role. OCO works in collaboration with caregivers, health care providers and other organizations to address the challenges that caregivers face so that all caregivers – regardless of age, condition, or geographic location – have access to the support they need.

We are recruiting for a Caregiver Link Worker, Eastern Ontario (Bilingual). This is currently a vacant position to support a growing program.

This role represents an exciting opportunity to be part of a growing program that aims to have a major impact on the caregiver experience in Ontario. You will be helping to enhance existing social prescribing principles to be more inclusive of the caregiver experience. The ideal candidate can work independently with minimal supervision, as well as on a team, and be able to carry out both program management and direct service delivery duties within the community. This is a remote first role that will require occasional travel to support in person presentations and deliver caregiver support within clinic or community settings.

Key Responsibilities

Reporting to the Manager, Caregiver Navigation and Workplace Programs, the Caregiver Link worker will maintain existing partner relations and establish new community-based social prescribing partnerships focused on supporting caregivers within integrated primary care and other community health settings.

You will be helping to expand on new models of social prescribing for caregivers in Eastern Ontario.

Provider Engagement

  • Identify and develop key partnerships with integrated primary care teams, community paramedicine or other community health services
  • Provide live in-person information sessions and lead in-person and virtual meetings with community partners
  • Connect with health and community service providers and work alongside them to identify and support caregivers
  • Virtually connect with Francophone providers in other regions outside of Eastern ON as needed.

Project Management

  • Identify potential partner sites and build caregiver social prescribing practices within each site with a specialization in supporting Francophone caregivers
  • Implement strategies, processes, and practices alongside partners for service delivery and referrals
  • Independently manage project deadlines, workflow, and daily service delivery requirements
  • Collect required data points and develop reports
  • Provide input to strategic planning for the program and organization as a whole

Direct to Caregiver Support

  • Manage incoming and outgoing referrals and communications in a timely manner following OCO service policies and protocols
  • Provide direct support to caregivers in-person, by phone, and email in French and English, listening to their needs and co-developing intervention options
  • Provide resources or program options for their care recipient as needed
  • Assist caregivers in navigating OCO programs as well as support within their community
  • Complete service readiness screening and program registrations as required

Qualifications

  • Minimum 5 years’ experience in primary care, community health settings, or clinical environments within community, health care, non-profit, and/or social service settings
  • Exceptional bilingual communication skills – verbally, in writing and in virtual environments
  • Highly skilled in supporting community members with complex needs
  • Excel at facilitation of workshops, focus groups, network meetings and outreach
  • Project or program management experience, managing deadlines and reporting requirements
  • Ability to develop and maintain partnerships with external groups and providers
  • Assessment capabilities to determine best service match
  • Ability to navigate community and publicly funded supports and services
  • Strong conflict resolution skills, ability to manage challenging situations
  • Comfortable working with vulnerable community members facing hardship
  • Experience with following privacy and consent guidelines
  • Self-starter, solution focused with demonstrated problem-solving skills
  • Knowledge of caregiver needs, the role of caregivers in the health and community care systems, and/or experience as a caregiver is an asset
  • Strong technical ability and proficiency in the use of computers and various software applications, in particular Microsoft Office, Dynamics 365, Survey Monkey, Eventbrite, Zoom, Ocean, Caredove
  • Ability to speak and write in French is an asset
  • Ability for occasional work after hours or on weekends to accommodate pre-arranged meetings or events
  • Ability to travel within Ontario, Valid G license and access to an insured and reliable vehicle,

Education

  • Bachelor’s degree in nursing, social work, social services or similar discipline
  • Registered Social Worker, Registered Social Service Worker, Registered Nurse or Registered Practical Nurse in good standing is an asset
