The “Tool Kit for Active Living with Diabetes + Calls” Program is a six week telephone series for anyone with Diabetes, in particular people with Type 2 and pre-diabetes. Anyone can register themselves for the program, no referral from a health care professional is needed. All participants will receive the “Living a Healthy Life with Chronic Conditions” companion book and “Exercises for Chronic Conditions” CD . We suggest that participants attend all six sessions of the program, where possible.
