Provincial Caregiver Link Worker, Full Time, Permanent

  • Virtual First Organization; Work from home-based office in Ontario, travel is required with occasional meetings and events within Ontario

The Ontario Caregiver Organization (OCO) exists to improve the lives of Ontario’s estimated 4 million caregivers; ordinary people who provide physical and/ or emotional support to a family member, partner, friend, or neighbour. OCO provides caregivers with one point of access to information, services and supports that empower and help enable caregivers to be successful in their role. OCO works in collaboration with caregivers, health care providers and other organizations to address the challenges that caregivers face so that all caregivers – regardless of age, condition, or geographic location – have access to the support they need.

We are recruiting for a Provincial Caregiver Link Worker.

This new role represents an exciting opportunity to be part of the navigation team, directly supporting caregivers across the province, assisting them in finding services to meet their needs and the needs of their care recipients. You will be helping to enhance our current referral pathways both internally and externally with community partners. The ideal candidate will be a high performing individual who can work independently with minimal supervision, as well as collaboratively on a team, and be able to follow and evolve protocols that support caregivers with complex needs who live across Ontario.

Key Responsibilities

Reporting to the Manager, Caregiver Navigation and Workplace Programs, the Provincial Caregiver Link Worker will:

Direct to Caregiver Support

  • Provide a confidential, warm, compassionate, and timely service to caregivers following OCO guidelines and protocols
  • Follow all privacy guidelines and practices
  • Provide direct support to caregivers by phone, email, or virtual meeting; listening to their needs and co-developing intervention options and next steps
  • Ensure all caregiver communications are clear and provide next steps and tailored information based on the needs identified in the form of communication that is most suitable for them (e.g. by phone/email)
  • Provide resources or program options for a caregiver’s care recipient as needed
  • Assist caregivers in engaging with OCO programs as well as support options within their community
  • Monitoring referrals and proactive follow up to ensure caregivers are connected to the service options provided and to determine if additional support is needed
  • Assess for caregiver distress and imminent risk, providing warm transfers to crisis or emergency services when needed

Administrative Duties

  • Maintain an extensive understanding of all OCO programs and services
  • Maintain a clear understanding of the common needs of caregivers and matching service options available across province
  • Research caregiver intervention options based on needs identified
  • Participate in related working groups, networks, and communities of practice
  • Work within the OCO team to build or enhance referral and registration systems
  • Assist with marketing and communication efforts, helping to develop content to promote OCO navigation and referral opportunities, as needed
  • Participate in meetings alongside other OCO team members to build external referral processes with partners as required
  • Manage incoming and outgoing referrals and/or completed registration forms from a variety of platforms including email, phone calls, Survey Monkey, Ocean MD, and Caredove in a timely manner
  • Complete service readiness screening and OCO program registrations
  • Complete referral forms to external services as required
  • Mail out materials when needed by caregivers or partners
  • Ensure accurate and timely reporting of data and performance indicators
  • Provide ongoing insights into the OCO team regarding caregiver needs
  • Maintain updated OCO service listings in databases across Ontario
  • Other duties as assigned

Qualifications

  • Minimum 5 years’ experience in navigation and direct support within community, health care, non-profit, and/or social service settings
  • Exceptional communication skills – verbally, in writing and in virtual environments
  • Highly skilled in supporting community members with complex needs
  • Assessment capabilities to determine best service match
  • Ability to navigate community and publicly funded supports and services
  • Strong conflict resolution skills, ability to manage challenging situations
  • Comfortable working with vulnerable community members, facing hardship
  • Experience with following privacy and consent guidelines
  • Self-starter, solution focused with demonstrated problem-solving skills
  • Knowledge of caregiver needs, the role of caregivers in the health and community care systems, and/or experience as a caregiver is an asset
  • Strong technical ability and proficiency in the use of computers and various software applications, in particular Microsoft Office, Dynamics 365, Survey Monkey, Eventbrite, Zoom, Ocean, Caredove
  • Ability to speak and write in French is an asset
  • Ability for occasional work after hours or on weekends to accommodate pre-arranged meetings or events

Education

  • Bachelor’s degree in social work, social services, nursing, or similar discipline
  • Registered Social Worker, Registered Social Service Worker, Registered Nurse or Registered Practical Nurse in good standing is an asset
