SUMMARY:Tools for Better Transitions Between Hospital and Home
DESCRIPTION:Transitioning between hospital and home can be stressful for family caregivers and their care recipients. In partnership with Ontario Health\, this webinar provides caregivers with information\, resources\, and tools to support patient transitions between hospital and home. We will hear lived experiences from two caregivers\, and Dr. Karen Okrainec\, a Clinician-Scientist and Assistant Professor at University Health Network and the University of Toronto.
