Devan is the Manager of Capacity Building & Talent Development at Gay Men’s Sexual Health Alliance.\n\nSpeakers:\n\nRyan Wannamaker is a Registered Nurse working within the Day Health Program (DHP) at Casey House. He is responsible for providing holistic client-centered care within an interdisciplinary health care team\, which is vital to improving the overall well-being of people living with HIV. In collaboration with DHP Physiotherapist\, Ryan has also developed an exercise program designed to address this population’s complex health challenges\, including aging with HIV\, multimorbidity\, and barriers to community exercise.\n\nShoshana Pellman has been an advocate for trans inclusion and care for over 15 years. Her story was published on Xtra Magazine and The Globe and Mail. She was interviewed on radio programs like Back in the Closet\, The Current\, and Sirius Radio 2011\, and she is also a presenter for Healthcare Providers\, Personal Support Workers\, and Allied Healthcare Providers. Shoshana is passionate about patient-centred care for the trans population.\n\nSue/Suza (pronouns she/her/ hers) is a Primary Health Care Nurse Practitioner at St. Michael’s Hospital Academic Family Health Team since 1994. She is an experienced nurse with over 30 years of experience and has been a consultant trainer with Rainbow Health Ontario teaching about transgender and gender-expansive medical and surgical primary health care. Suza’s incredibly supportive work environment has provided the time and space needed to develop this clinical expertise. Learn about the signs of guilt and the common themes often associated with guilt as a caregiver. This session will help you differentiate between two coping pathways: (1) thoughts and (2) emotion. Too often we face cognitive distortions\, the sneaky ways your mind convinces you of something that isn’t really true. Learn to identify and unpack these cognitive distortions that you may be having as a caregiver like “I should be able to care for her on my own at home” or “I’m a failure as a daughter.” We will question these negative thoughts and ask\, “is this 100% true?”. She lives in Toronto and spends her time advocating for an integrative approach to healthcare. She speaks to a variety of audiences about the ways in which the body and mind are connected. \n\n\nMichal Goldstein\n\nMichal Goldstein (RSW) is a Registered Social Worker working at Dynamic Health Clinic in Toronto where she sees adults for individual\, couples and group counselling. She is also the founder of Creating Connections\, a play-based therapy practice in Toronto\, where she works with children suffering from mental health and behavioral challenges. She has over a decade of experience working with caregivers to support them in improving their ability to cope with the challenges inherent in caring for someone else. Michal has developed numerous workshops and support group programs utilizing knowledge of interpersonal neurobiology and mindfulness techniques to support individuals suffering from physical health\, mental health\, and parenting challenges. She is passionate about bringing evidence-based practice to the forefront of the clinical social work field. Michal knows both first-hand and from professional experience\, the challenges that come with caring for someone with a physical and mental health illness and is grateful for the opportunity to bring more awareness\, education and support to this community. \n\n\nShoshana Kulik\n\nShoshana Kulik (RSW) is a registered social worker at the Dynamic Health Clinic. She is caring and empathetic and has experience working at Circle of Care with the elderly. Shoshana has a strong awareness of the possible physical and emotional challenges that can arise from the responsibility of taking care of a loved one. With extensive mental health experience\, she understands how the strains of caregiving can affect one’s mental health and that sharing ones’ struggles can give some relief from the stresses of continuous care for others. Shoshana utilizes the Strengths Based Model which capitalizes on a client’s strengths to foster growth. DESCRIPTION:In this webinar\, we will cover two main topics. The first is a discussion of the fundamental aspects of advance care planning (ACP) as it applies to you and to the people you care for. We will discuss what ACP is\, why it is important and how to begin advance care planning discussions. We will also discuss the role of a substitute decision-maker (SDM). The second topic will include a discussion of important concepts in caring for people with advanced illness\, such as “what does resuscitation status really mean?” and “when should I have palliative care involved?” and “what is the difference between care in a hospital versus a palliative care unit or hospice?” \nThis webinar is presented by Dr. Leah Steinberg\, a palliative care physician at Mount Sinai Hospital. She currently teaches palliative care to all level of learners and is involved in research in creating models for integrating palliative care into chronic disease management. Dr. Steinberg is also a member of the Advance Care Planning\, Goals of Care\, and Health Care Consent Clinician Advisory Team at Hospice Palliative Care Ontario.
