Including Caregivers as Part of the Care Team

There is strong evidence that shows that when caregivers are included and empowered as part of the care team, it improves health outcomes and enhances the experience for patients, clients, residents, families and care providers.

Individual care providers and health care organizations both have an important role to play in embracing the experience, expertise and value of caregivers.  Many resources and tools are available to help:

Caregivers as Partners eLearning for Healthcare Providers This free-of-charge education series is designed to support health providers on how to improve the caregiver experience and include caregivers as partners in care.  The series offers practical, tangible tips for health care providers though a series of 20 minute modules. A mental health and addictions series is also available to address the unique challenges involved with this caregiving journey.

NOTE: The Ontario Caregiver Organization (OCO) will be offering a new way to access these courses beginning May 1, 2021. There will be no access to the courses from April 1 to May 1, 2021 as this transition occurs, and all previous registration information will become obsolete.

How does this affect you?

  1. If you already completed a CAP course and received your certificate of completion, this change will not affect you and you will not have to do anything.

  2. If you started a course and have not finished it, please do so by March 31, 2021. Your registration data and course progress will not transfer to the new LMS. It is a different platform and therefore will require new registration to take a course there.

  3. If you registered for a course, but did not start it yet, we encourage you to consider re-registering on the new LMS after May 1st and begin the course then, if you do not think you will have time to complete the course between now and March 31, 2021, on the current website platform.

The URL for the new LMS is https://learning.ontariocaregiver.ca/

Currently, the only course on the LMS is the new Caregiver 101 eLearning module targeting new unpaid, family caregivers.  It focuses on some of the common challenges a caregiver might expect, including the emotional journey, how to prepare, and supports that are available through OCO. Should you wish to refer Caregiver 101 to caregivers, please do!

Caregiver ID:  Recognizing Caregivers and Facilitating their Role A valuable tool that visually recognizes family caregivers and helps to facilitate their role as partners in care.  Caregiver ID is a strong demonstration of the organization’s commitment to family caregivers.

Partners in Care Pandemic Tool Kit This tool kit provides hospitals, long term care homes and other health care settings with practical resources that can help facilitate the safe presence of family caregivers and enable the essential role they play in patient and resident care during COVID-19.

Supporting Transitions through Discharge Planning Returning home or moving from hospital to another setting is often a challenging time for patients and family caregivers.  Transitions can be easier when caregivers are involved in discharge planning.

Understanding Your Care Team Often, a circle of care involves many different healthcare providers and it can be challenging for patients and their caregivers to remember the role of each provider on the care team.  This tool can help caregivers to better understand roles on the care team so that they can meaningfully participate as partners in care.

Caregiver Pledges, Rights & Commitment Statements  These tools visually acknowledge a commitment from organizations to caregivers as care partners. They provide a clear set of expectations from staff and for caregivers demonstrating a commitment to including caregivers as valuable members of the care team.
